Signal one: the same mark lives everywhere
Teachers, departments, administrators, leaders, and families may each hold a different copy of a mark or report. People then spend time reconciling rather than reviewing learning evidence.
Inventory gradebooks, spreadsheets, exports, email attachments, paper notes, reports, and integrations. Name the authoritative record and the owner of each correction.
Signal two: calculations depend on memory
If weighting, rounding, grade boundaries, missing values, resits, or special consideration live in one person’s spreadsheet, a change can affect a cohort without a visible version or approval.

Review selection, adjustment, disagreement, approval, notification, history, access, export, and archive. Include subject, cohort, campus, grading period, and assessment context.
Signal four: exceptions dominate
Missing submissions, late work, absent learners, transfer, withdrawal, duplicate entry, report correction, access failure, and outage become daily work rather than occasional exceptions.

Make the next improvement testable
Use this guidance to improve one bounded part of signals teams have outgrown school exam and gradebook software. Name the owner, assessment or gradebook record, evidence, correction route, and review date so staff can apply it consistently.
