Start with the result decision

Name the decision the system should support: set an assessment, record evidence, apply a scale, moderate a judgement, approve a result, publish a report, or correct an error.

Record subject, cohort, campus, grading period, owner, audience, timing, authoritative source, acceptance test, and consequence of an incorrect or late value.

Look for data risk

Check unclear assessment components, duplicate learners, wrong subject or cohort, stale grading periods, hidden formulas, invalid ranges, ambiguous statuses, unapproved changes, and reports that cannot be reproduced.

Make risk actionable

Write risk, cause, affected record, consequence, likelihood, owner, control, evidence, due date, trigger, and residual risk. Review completeness, timeliness, moderation, corrections, staff effort, support demand, report confidence, and access exceptions at 30, 60, and 90 days.

Make the next improvement testable
